We are Age Central

We are an independent, 501 (c) (3), not-for-profit organization with a 49-year history of providing service to older persons and their caregivers in Fulton, Marshall, Peoria, Stark, Tazewell and Woodford Counties. Our guiding principles are that we:

Believe in the independence and dignity of all persons;
Empower each person to exert control over his or her own life;
Increase access to needed services of quality;
Target services to older persons with the greatest economic and social need;
Develop services based on the needs or priorities of older persons, persons with disabilities, and their caregivers.

What we facilitate

Caregiver Support

The Caregiver Support Program at Age Central provides vital resources, counseling, and respite services to family caregivers, helping them manage stress and sustain their caregiving responsibilities. Through evidence-based tools like TCARE®, and support for programs such as Grandparents Raising Grandchildren, Age Central equips caregivers with personalized assistance and meaningful relief to improve both their well-being and that of their loved ones.

GUIDE Dementia Care

Caring for someone with dementia can be overwhelming. Age Central’s GUIDE program offers personalized care coordination, caregiver support, and respite services—at no cost to eligible Medicare beneficiaries. With a dedicated care navigator and ongoing guidance, families gain clarity, confidence, and support at every stage.

Veteran-Directed Care

Age Central’s Veteran-Directed Care (VDC) program allows eligible Veterans to direct their own in-home support through a flexible monthly budget funded by the U.S. Department of Veterans Affairs. Veterans choose their caregivers, set schedules that fit their lives, and receive guidance from Age Central to remain living independently in their communities.

Outreach

The Outreach Program at Age Central raises awareness about vital services and benefits available to older adults, caregivers, and the broader community through events, partnerships, and direct engagement. It also supports fraud prevention efforts through the Senior Medicare Patrol (SMP), helping individuals detect and report Medicare fraud while ensuring equitable access to resources across the six-county service area.

Nutrition

The Nutrition Program at Age Central provides older adults with nutritious meals through congregate dining sites and home-delivered options, helping reduce food insecurity and promote health and social connection. It also includes wellness checks, nutrition education, and seasonal support through the Senior Farmers’ Market Nutrition Program, fostering independence and healthier lifestyles for aging individuals.

Caregiver Training with Trualta

Age Central is partnering with Trualta to bring additional support to family caregivers. Details on how to take part are coming soon. In the meantime, the Caregiver Support Program offers counseling, respite, and help finding the right services for families across our six counties.
